Singles Style-Up

Participant Waiver, Release of Liability & Media Consent

By submitting this application for Singles Style-Up, I acknowledge and agree to the following:

1. Voluntary Participation

I understand that Singles Style-Up is a voluntary style and personal development experience facilitated by Singles Mingle LLC and its partners, including professional stylist services and dating/communication coaching. I am choosing to participate freely and understand that selection is not guaranteed.

If selected, I agree to participate in good faith, arrive on time, and engage respectfully in all scheduled sessions and the live reveal event.

2. Assumption of Risk

I understand that this experience may include:

• Personal styling sessions

• Coaching conversations related to dating and communication

• Participation in a live, in-person and streamed event

• Audience Q&A and public discussion

I acknowledge that sharing personal experiences may involve emotional vulnerability. I assume full responsibility for my participation and understand that Singles Mingle LLC, its owners, partners, contractors, and venue hosts are not providing therapy, legal advice, or medical services.

I voluntarily assume all risks associated with my participation.

3. Release of Liability

I release and hold harmless Singles Mingle LLC, its owner, employees, contractors, partners, sponsors, venue hosts, and affiliates from any and all claims, liabilities, damages, losses, or expenses arising from or related to my participation in Singles Style-Up, including but not limited to personal injury, emotional distress, property loss, or dissatisfaction with services provided.

4. Media & Publicity Consent

I understand that Singles Style-Up includes photo and video capture for promotional, marketing, and streaming purposes.

If selected, I grant Singles Mingle LLC permission to:

• Photograph and film me

• Use my name, image, likeness, and voice

• Share content on social media, websites, email marketing, and future promotional materials

• Stream the reveal event live

I understand that this content may be used indefinitely without compensation.

I also understand that the photo uploaded with my application will not be shared publicly and is used only for selection and representation purposes.

5. Code of Conduct

I agree to:

• Treat all participants, facilitators, and attendees with respect

• Engage in discussions in a constructive and mature manner

• Represent myself honestly during the application and experience

Singles Mingle LLC reserves the right to remove any participant who engages in disruptive, inappropriate, or unsafe behavior.

6. Application Fee Acknowledgment

I understand that the $5 application fee:

• Is credited toward participation if I am selected

• Is non-refundable if not selected

• May be applied toward attending the April 11 event if I choose

I acknowledge this fee helps cover review time and supports the selection process.

7. Agreement

By submitting my application, I confirm that:

• I am at least 18 years of age

• I have read and understand this waiver

• I voluntarily agree to all terms listed above

Electronic submission of this application constitutes my legally binding agreement.
